Inside ENLIGHTEN: The Phase 3 Trials That Will Test Eloralintide’s Next Chapter

ENLIGHTEN is a collection of registered late-stage studies exploring eloralintide in different populations. Here's what the individual trials are designed to investigate.

The Next Eloralintide Story Has a Name

Clinical research has an interesting habit.

Just when a major trial announcement attracts everyone’s attention, the real scientific work moves into a much longer and less glamorous phase.

Larger studies. Additional populations. More safety monitoring. Months of follow-up. And enough registry paperwork to test almost anyone’s patience.

For eloralintide, that next stage includes the ENLIGHTEN clinical development program. This article describes what the registry records say about it as of October 11, 2026. It does not report any Phase 3 results, because none have been posted.

ENLIGHTEN Is Not One Trial

The registrations reviewed here describe five separate Phase 3 studies of once-weekly eloralintide compared with placebo, each in a different population. No registry record labelled ENLIGHTEN-5 was found at the verification date, so this list should not be read as a complete count of the program.

Each study asks a different clinical question. That matters because an investigational compound can behave differently depending on the population and the endpoints chosen.

The trials should be followed individually, not described as one enormous study.

ENLIGHTEN-1: Obesity Without Type 2 Diabetes

ENLIGHTEN-1 is a randomized, double-blind, placebo-controlled Phase 3 study in adults with obesity or overweight who do not have type 2 diabetes. [1] The registry lists an enrollment goal of 1,980 participants and a status of recruiting. [1]

The main phase lasts about 75 weeks. Participants with prediabetes continue into an extension phase of about two more years. [1] The primary outcome is the percent change in body weight from baseline to week 64. [1]

The registered design shows what researchers intend to investigate. It does not establish what they will find.

ENLIGHTEN-2: The Type 2 Diabetes Population

ENLIGHTEN-2 enrolls adults with obesity or overweight and type 2 diabetes, with a listed goal of 1,035 participants. [2] It is also recruiting, and participation lasts about 75 weeks. [2] The primary outcome is again the percent change in body weight at week 64. [2]

The difference from ENLIGHTEN-1 matters. Type 2 diabetes adds metabolic considerations, and the results have to be read within that study population. The registry also excludes people who recently used certain diabetes medicines, including GLP-1 receptor agonists and insulin. [2]

A result in one group cannot simply be assumed to hold in another.

ENLIGHTEN-3: Obstructive Sleep Apnea

ENLIGHTEN-3 is a master protocol supporting two independent studies in adults with moderate-to-severe obstructive sleep apnea and obesity or overweight. [3] One study enrolls people who are unable or unwilling to use positive airway pressure (PAP) therapy. The other enrolls people who have used PAP for at least three months and plan to continue. [3]

The registry lists 800 participants and two primary outcomes at week 64: percent change in body weight, and change in the apnea-hypopnea index, a measure of breathing interruptions during sleep. [3]

A registered study does not mean eloralintide has shown a benefit for sleep apnea. That is what the trial is designed to find out.

ENLIGHTEN-4: Knee Osteoarthritis

ENLIGHTEN-4 is also a master protocol supporting two independent studies, in adults with obesity or overweight and osteoarthritis knee pain. [4] The registry lists 900 participants. [4]

Its two primary outcomes at week 64 are the percent change in body weight and the change in the WOMAC pain subscale score, a standard questionnaire about knee pain. [4] Whether pain changes in any meaningful way will be answered only by the completed analyses.

ENLIGHTEN-6: Persistent Obesity During Incretin Treatment

ENLIGHTEN-6 asks a different question. What happens when eloralintide is studied in people with persistent obesity or overweight who are already on stable weekly incretin therapy, with or without type 2 diabetes? [5]

The registry describes a Phase 3 study comparing eloralintide with placebo under those conditions, with a listed goal of 900 participants and about 80 weeks of participation. [5] The primary outcome is the percent change in body weight at week 64. [5]

Adding a second pathway does not automatically improve outcomes. The trial has to show whether the investigational approach produces a measurable difference and has to characterize the adverse events.

What Phase 3 Actually Means

Phase 3 trials are generally larger, later-stage studies meant to provide evidence relevant to regulatory evaluation.

But the phase number is not a report card. A registered Phase 3 study can produce positive, inconclusive or negative findings. Recruitment changes. Protocols get amended.

As listed on October 11, 2026, primary completion dates run from January 2028 for ENLIGHTEN-2 to June 2028 for ENLIGHTEN-6, and the ENLIGHTEN-1 extension has a listed completion date of July 2030. [1][2][3][4][5] Those are estimates until the studies actually finish.

A Separate Combination Program

The five registrations above compare eloralintide with placebo. Lilly has separately announced plans to start Phase 3 trials of EloraTZP, an investigational combination of eloralintide and tirzepatide, in the fourth quarter of 2026. [6]

That combination program is a different research effort and is not described by these registry records.

The Bottom Line

ENLIGHTEN matters because it widens the clinical questions being asked about eloralintide. It covers different populations, different primary outcomes and different background treatments, all under registered protocols.

The results will help determine what the evidence ultimately supports. Until then, trial registration and recruitment should be described for what they are.

Plans for scientific investigation. Not proof that the answers are already known.
